The aim of this project is to incorporate an intensive case management system, consisting of tailored follow up with patients and their families alongside comprehensive patient education, into existing Diabetic Retinopathy grading and treatment programmes. The purpose of this is to improve knowledge and promote care-seeking behaviour by working with patients and their families to overcome real and perceived barriers to seeking care. It is expected that by increasing the number of patients who receive (and pay for) Diabetic Retinopathy treatment, health facilities will ultimately see the financial benefit of initial investment in an intensive case management system.
